• DIAGNOSTIC EXAMS: When a health provider discounts fee 10% or more for one of the following six categories per member per year, Fund will pay $20 of fee, for a member who has paid $50 or more annual membership: DENTAL EXAM, PERIODONTAL EXAM, EYE EXAM, GENITO-URINARY EXAM, HEARING EXAM, MEDICAL EXAM
